Reprojected shapefile has 50m offset

Reported by Siebe Bosch (HydroConsult):

The attached zip-file contains a shapefile in EPSG:28992 (Amersfoort) when loaded with tiles in Amersfoort as well it looks correct:

When creating a new map and setting the map projection to EPSG:3857 and opening the shapefile again the reproject box appears.
When you select reproject you can see it is several meters off:

ogr2ogr -t_srs "EPSG:3857" "Afvoergebied_De_Tol-r.shp" "Afvoergebied_De_Tol.shp"
has the same offset.

ogr2ogr -s_srs "EPSG:28992" -t_srs "EPSG:3857" "Afvoergebied_De_Tol-r2.shp" "Afvoergebied_De_Tol.shp"
is correct. In this statement I added -s_srs.

Perhaps this can be changed in the reprojecting process: When a EPSG code is recognized use -s_srs, this will also solve the issues with dialects.
